Health Assessment Program
A Partnership between the
Sullivan Senior Center
and the
Community Health and Wellness Center
 The Health Assessment Program provides comprehensive health care services to residents of the Northwest Corner who are age 60 or older.
The purpose of the program is to provide health promotion and prevention services, early detection of illness, education and counseling to promote a healthy life style.
Services provided include:
Blood Pressure, Pulse, Respirations, Height and Weight
Ear Assessments and Irrigations
Health History
Physical Examination
Pap/Pelvic/Breast/Exam
Prostate Exam
Health Education
Lab Services: Specific bloodwork is individualized by diagnosis (lab fee coverage depends on insurance carrier).
Diagnostic Imaging: Specific diagnostics are individualized by diagnosis.
Where: Sullivan Senior Center, 88 East Albert Street, Torrington --- Health Office Bldg. B
Scheduling: Walk in hours 9:00-noon on Tuesdays. Call 860-489-0931 to schedule an appointment.
Ami Marshall, APRN, MSN, nurse practitioner is responsible for the health assessment, intervention and referral services.
Kelly Roma assists the nurse practitioner and is a certified medical assistant and phlebotomist.
Additional Services:
Deborah Reardon, LPN, is available on Thursdays, noon - 1:00 pm for Blood Pressure Screening.
Foothills VNA Foot Clinics are held monthly on Fridays, check the newsletter for date, appointments are required. Please call 860-379-8561.
Lions Low Vision Screenings are held monthly on thursdays, check the newsletter for date, appointments are required. Please call 1-800-676-5715.
Jack Felix, BC-HIS,AC, visits quarterly for Hearing Screenings, Appointments are required. (2011 Dates - April 20, June 15, Sept 21 & Dec 21).
